PLEASE NOTE: When payment is completed, you will be given access to the training portal. There is a folder with in the portal called "Basic Training Verification". You must upload a copy of your EMDR Basic Training Certificate of Completion before you will have access to the rest of the training materials. Per EMDRIA requirements, you must submit verification that you are EMDR trained prior to receiving your certificate of completion for an EMDR advanced training. PARTICIPATION AGREEMENT
Please review the following information carefully before submitting payment and registering for this training. In addition to this agreement, you are responsible for reading and understanding the

By submitting your registration and payment, you acknowledge and agree to all terms outlined in this agreement and the full LK Institute Training Policies.
- In order to be eligible for participation in this advanced training, I understand and acknowledge that I have completed an EMDR Basic Training, including the required consultations, and I am EMDR trained.
• I understand and acknowledge that I must submit a scanned copy or photo of my EMDR Basic Training Certificate of Completion to verify that I am EMDR Trained prior to the training start. I understand and acknowledge if I do not submit the EMDR Basic Training
Certificate of Completion prior to the training start date that I will not be permitted to attend and will not qualify for reimbursement.
• I understand and acknowledge that all participants are expected to maintain a spirit of cooperation and mutual support for all in the training. I understand and acknowledge that attention seeking and/or conflict- generating behavior is not conducive to group learning. I understand and acknowledge that the training director has the right to dismiss anyone who is disruptive from the training at any time.
• I understand participants must attend the training in full to receive the continuing education credits. This includes, but is not limited to applicable attendance, learner assessments and evaluations. Participants will not receive partial credit for partial attendance.
• I understand and acknowledge that in order to assure confidentiality of personal and clinical information, audio/ video recording by participants is not allowed. I understand and acknowledge that it is expected that all participants shall maintain the highest ethical standards of confidentiality regarding all personal and clinical information shared by others in this training. I understand and acknowledge that failure to maintain confidentiality shall be treated as a professional ethics issue and may result in immediate dismissal from the training program.
• I understand and acknowledge that should I attend the entirety, or a portion, of this training virtually, I am required to keep my camera on at all times, unless indicated otherwise by the trainer. Per EMDRIA requirements and our continuing education provider, virtual trainees must have their cameras turned on so that training faculty can monitor attendance, visual cues, facial expressions, etc. I understand and acknowledge that should I keep my camera off when indicated to keep my camera on, I may not qualify for continuing education credit and may not receive my Certificate of Completion.
• I understand and acknowledge that the trainer, nor other therapists/ participants, shall be construed as providing supervision to a participant on any specific cases. I understand and acknowledge that while clinical possibilities will be discussed, and ideas will be shared in relation to the clinical situation presented, I am solely responsible for the clinical management of my client(s)/patient(s) and I am expected to exercise my best judgment in all relevant clinical matters.
• I understand and acknowledge that whatever information is presented in these sessions is to be kept confidential among the participants both during and after my participation in the group. I agree to avoid disclosure of client’s names or other identifying information in making verbal presentations and in sharing written documentation of the client sessions.
• I understand that I may have the opportunity to volunteer for a live demonstration during this training, and that participation is completely voluntary and not required for completion. If I
choose to participate, I accept full responsibility for monitoring my own comfort, using personal boundaries, and deciding what to disclose. I understand I can stop at any time, decline to answer questions, or disagree with clinical perspectives without consequence. If I do not feel confident in my ability to self-monitor or assert verbal boundaries, I agree not to volunteer. I acknowledge that the trainer is not acting as a therapist or supervisor and that demonstrations are for educational illustration only—not clinical assessment or supervision.

- As part of this training, there may be opportunities to engage in practicum exercises to practice clinical techniques with other training participants in a structured learning environment. Participation is voluntary and is not a requirement for training completion. All participants are expected to uphold professional ethical standards in accordance with their licensure board and the ACA Code of Ethics. Practicum exercises are for educational purposes only and are not a substitute for clinical supervision or therapy. Trainees are responsible for using clinical discretion, maintaining confidentiality, and ensuring that their participation does not compromise their own or others' well-being. • I understand that video, audio, transcript, or case materials related to client demonstrations, participant demonstrations or case presentations may be shared during the training with appropriate ethical safeguards and informed consent from those involved. I agree to maintain strict confidentiality regarding any such content, and will not disclose, distribute, or discuss these materials outside of the training setting.
- I understand that if I am an employee of LK Institute, there are additional ethical, professional, and organizational considerations that may apply to my participation in this training. These may include dual relationships, boundary complexities, or potential conflicts of interest. I agree to carefully review LK Institute’s most current Training Policies, which outline expectations for employees attending training in any capacity, including risks, benefits and limitations. - I also understand that participation in LK Institute trainings—whether as a learner, observer, or demonstration volunteer—carries potential benefits, such as professional growth, exposure
to advanced clinical frameworks, and experiential learning. However, risks may include emotional discomfort, vulnerability when engaging in discussion or live demonstration, or challenges related to role navigation if I am also a staff member or amidst professional colleagues, coworkers, referral sources or coworkers. Limitations include that this training is not a substitute for supervision, therapy, or clinical consultation, and that case material
discussed is illustrative, not diagnostic or prescriptive. Clinical outcomes cannot be guaranteed in terms of application to techniques and material covered in this course. I accept responsibility for setting and maintaining appropriate boundaries, seeking clarification where needed, and using professional discretion in how I participate. Referrals for mental health providers will be made available upon request from the participant. - I understand that I am responsible for being mindful of potential power dynamics or professional implications when choosing to engage in
discussions or demonstrations. Disclosures made during training may be visible to colleagues, and I understand I should exercise discretion and self-monitoring regarding what I share. Participation offers the benefit of enhanced clinical skills and applied learning; however, it also carries risks—such as emotional activation, professional vulnerability, or
unintended disclosures—and limitations, including that insights are educational and not clinical directives, and training participation may not be suitable for those unable to set or express personal boundaries in a group setting. - I understand that this advanced training qualifies for 17 NBCC hours. LK Institute has been approved by NBCC as an Approved Continuing Education Provider, ACEP No. 7464. Programs that do not qualify for NBCC credit are clearly identified. LK Institute is solely responsible for all aspects of the programs.
- I understand that LK Institute is an EC Provider (#19005) and is approved for 18 EMDRIA Advanced Training credits (Approval #19005-DL09). Eligibility for EMDRIA Credits is restricted to those who have completed an EMDRIA- Approved Basic EMDR Training.

Training Policies Agreement & Acknowledgment Clause
By registering for and participating in this training, I understand and agree to the statements listed in this registration form/participation agreement and abide by the most current version of LK Institute’s Training Policies, in addition as publicly posted at: https://lkinstitute.com/training-policies
I understand that:
• These policies are legally binding and updated periodically to reflect current standards and compliance with EMDRIA and ACA ethical guidelines.
- The most recent version will always be accessible via the above link and/or within the participant portal. It is my responsibility to review the current version of the policies prior to each training I attend.
• My continued participation in any training constitutes acceptance of the most current version of the Training Policies, even if updated after registration but before training begins. I am not required to receive individual notification of updates to remain bound by them.
I understand that LK Institute’s Training Policies exist to:
• Ensure ethical protections for all participants, including student learners, supervisees, and employees attending training
• Maintain clear boundaries regarding dual relationships, conflicts of interest, and roles between trainers, observers, and attendees
• Outline responsibilities around confidentiality, participation, feedback, and trainee conduct
